The Chicago Passport Agency requires that all applications submitted by American Visa Service include some proof-of-departure, even if no such travel plans exist.

As of February 20, 2015 - you must provide a flight itinerary or copy of tickets for any travel in less than 9 days. The letter template below will only work if you are traveling in more than 9 days. For travel in less than 9 days, you MUST present a flight itinerary.

Use the following template for the a letter if you are departing in 9 days or later and change the items in red to fit your situation. If you are traveling for business the letter should be printed on Company letterhead and signed by someone other than the traveler. If the travel is personal, the traveler can sign the letter.
